This Personal Information and/or Sensitive Personal Data or Information shall be collected by us from the concerned network provider(s) through our hospital portal.

Further, for the purpose of processing a reimbursement medical claim, we collect certain Personal Information and/or Sensitive Personal Data or Information about you including but not limited to physical originals / photocopies of medical and health reports, discharge summaries, invoices etc. as issued by network provider(s) or non-network provider(s) to you and as submitted to our designated offices by you.
